Summary
Ren is in the middle of university while juggling multiple jobs, Akechi has come back from whatever not-death he went through, and the Metaverse is a vague buzz at the back of everyone's minds. It would be nice if things stayed stable, but Ren has never lived a life anywhere approaching "stable" or "normal" or "manageable". Which is to say, Ren finds an anomaly in Mementos and accidentally fucks things up when he tries to investigate it with the other Phantom Thieves.
Six years after shooting Yaldabaoth in the head, Amamiya Ren meets a stranger, finds an intruder, and has to fight off Shadows in the Metaverse again. Also, he reconciles with the person who tried to shoot him in the head.

“I can’t say that I have heard about this…Joker,” Goro says. “But then again, I haven’t exactly been able to keep up with current affairs.”
Ann cringes. “Well, he’s, uh…very mysterious anyway. No one knows much about him. He showed up about a hundred years ago, but none of us—” she waves a hand to encompass all of heaven, “—have ever seen him directly. We’ve just heard rumors.”
After more than two centuries of punishment for an unforgivable crime, Akechi Goro, the God of Justice, has been granted another chance. Determined not to waste it, he spends his time on earth, dealing with incidents that other gods can't or won't handle themselves, all while trying to leave his painful past behind him. His hopes for a peaceful existence are shattered by the appearance of Ren, a mysterious young man who seems abnormally interested in him and who awakens memories that Goro would rather stay buried.

Summary
The Holy Grail couldn't beat the Phantom Thieves, so it did the next best thing. It used all its power to seal them away inside itself, then retreated to the bottom of Mementos to lay dormant, slowly regathering its strength to erase them for good. Akira managed to escape before it was too late, but the rest of the Thieves weren't so lucky. To jailbreak a living prison formed from the body of a god is an impossible task for the Trickster alone. He can't do it without his friends.
But luckily, there's more than one group of Phantom Thieves within reach.
With the help of Igor and Lavenza, Akira crosses to a world very much like his own but different in unexpected ways. With the Phantom Thieves of this reality on his side, Akira has a shot at saving his own. There's just one issue: These alternate Thieves have their own false god to deal with first, and Akira can't let them fall for the same trap that his own did. He needs to help them on their journey, and he somehow needs to do it without catching the eye of the second Holy Grail dwelling within this world's collective unconscious.
