Gaius immediately assumed the worst, but it was an easy mistake to make. The grim expression on Merlin’s face as he stalked into the infirmary did not particularly shout ‘success.’ Neither did the way he flung his pack and jacket in the general direction of his bed without bothering to actually enter his bedroom, then spun on his heel and went straight to the storage shelves, glowering at the jars and bottles as if daring one or more of them to make a hostile move. They didn’t, and he cursed, not quite under his breath.The old man flinched. “Merlin…” he started. “What happened?”
“Oh, you know,” said Merlin over his shoulder, with a sarcastic flip of a hand. “Same old, same old. Woke up, had breakfast, summoned a goddess; nothing worth mentioning.”

Summary
After Merlin loses a patient, he finds that he can't stop himself from blaming Arthur for not doing enough to protect the druids. Unfortunately, the situation only worstens as Arthur's inaction and an innocent death starts off a chain of events that threatens not only Camelot, but all of Albion.
Canon up until "With All My Heart;" story begins approximately a week after they return to Camelot at the end of the episode.

“Then look in the bloody mirror!” For a moment, Merlin didn’t realize that he’d spoken aloud; he barely recognized his own voice, vicious in a way that Merlin had never allowed himself to sound in Arthur’s presence.

They write in your book all the time, Gwaine had said. Now Merlin turned page after page, each one covered with his own handwriting, but no longer his alone -- Arthur's neater, noble-born script filled corners and edges and even the spaces between lines, with Gwen's careful letters occasionally interspersed. Sometimes their words spilled over into the next page. Sometimes he had to pull his light closer to make out the words, blinking against suddenly blurring vision.

And Merlin was neither careful nor skilled -- he was clumsy and foolish and stupidly, infuriatingly prone to putting himself in danger for Arthur's sake.
So yes, he'd always known that he might lose Merlin one day, but he didn't think about it. For years he had blindly, determinedly not thought about it.

Arthur stares at Guinevere trying to make sense of things, this was not his wife, this person was not his Gwen; because Guinevere- Gwen would know that Merlin doesn't prefer the company of any girl that's not her, that their husband has all people he loves in Camelot and does not have to go to outlying villages.
