I'm a little confused though... Is Deku sad because Bakugou won't say out loud that he loves him? Or is he sad because they can't make their relationship public? Is it because Bakugou doesn't want it to be public? Or social homophobia reasons?

In my mind, the reason was mostly because of their inability to be public. The Deku in my headcanon is always freely affectionate and expressive. He wanted to be loud and proud with his love. This was, unfortunately, not possible in this fic's world setting due to homophobia. I also imagine Bakugou to be the opposite of Izuku - he's not as expressive, and in fear of the public's retaliation towards their relationship, he clammed up more and refused to be overly affectionate with his partner, much to Izuku's dissatisfaction. In short, the reason is pretty much a mish-mash of everything you've guessed haha.
