As someone who recently suffered a miscarriage, I think the only thing you got wrong was the fact that it's not over in one night. The miscarriage is like a heavy menstrual cycle that lasts, at least in my case, for a little over one week. That why I believe it's called bleeding out because you bleed out everything, including the mass of the fetus.
So iruma would definitely have some question after waking up, especially if his miscarriage was similar to how a female miscarriage occurs.
Either way I loved each alternative ending, they each gave me different feels.

I just wanted to come back and say thank you for your condolences, bit most importantly I just wanted to let you know that you did not offend me or trigger me in any kind of way.
I know some authors have been chased out of fandoms because people get angry or upset about what an author writes. So I just wanted to let you know that your tags and your notes definitely let me know what to expect of each ending, and which chapter to skip should I wanted to avoid it.
Smh, I know some people only want to read fluff and feel good stuff, but I always believe art should imitates life, and stuff like miscarriage, teen pregnancy, accidental pregnancies and whatever else triggering and painful or offensive happenstance are apart of real life. I like reading about my anime characters experiencing real life situations and problems.

Yes, unfortunately lately a somewhat unstable and anxious climate has been created for writers within fandoms, as if we had to carry out certain political agents or simultaneously satisfy the different expectations of different people with different personal sensibilities. I do my best to insert the necessary tags and trigger warnings, but for the rest I want to write what I want because, in the end, I'm writing to satisfy my own thirst for new Mairuma FFs. The fact that they are read, possibly appreciated by other people is one thing and I'm really happy when it happens, but I don't want to be tied to other people's expectations because it would kill my desire to write and imagination. As you have well highlighted, I intend to write more slices of life scenes or things that can happen in future fics. I have already touched on topics such as gender dysphoria, menstruation, the first sexual intercourse, fever, internal inflammation, etc. These are all normal things that you shouldn't be afraid to write about, in fact by doing so I hope to give a minimal contribution to the extension of Mairuma's world.
