Kakashi is intimately familiar with loss.
He’s lived and breathed it since the ripe age of six. Scrubbed blood off floorboards, more blood from his hands.
He’s a curse to those who get close to him, used to people breaking down his barriers before leaving or dying in some horrific manner. He’s kept a count, reasoned it down to a science.
So when Shisui dies and Itachi leaves, the first thought that came to his mind was simple—
Ah. It happened again.
But when Shisui returns a week after the Uchiha Massacre, banged up, yet still breathing, Kakashi isn’t sure what to think.
His ghosts don’t usually come back.

As soon as the name appeared in his mind, Itachi could no longer pretend that this was untrue, that the ghost was merely an illusion and not a memory, not a person. Truly, this should not be happening. Itachi knew that the clearer the ghost became, the more he recognized it. The more the line between reality and fantasy blurred, the worse it became.
For Itachi, ghosts didn’t exist—only illusions. Itachi, a master of Genjutsu, was accustomed to illusions and fantasies, and it wouldn’t be strange if his fractured mind began unconsciously casting Genjutsu on itself. It wouldn’t be strange if his wounded mind started playing tricks on him. It wouldn’t be strange if his conscience led him to madness... and yet...
Let’s start from the beginning... Itachi has a ghost, and the clearer that ghost becomes every time he sees it, the greater the catastrophe.

Cold. It was cold. The current of the river carrying him strongly, the water hitting his body. That was all Shisui could remember.
He was dead, that much was understandable. He was light, too light, like he no longer had a body to call his own. And he could see, Shisui had no eyes left, one stolen and the other in Itachi's possession, so the fact that he could see was surprising.

Or, Shisui Uchiha as a ghost, waking up (sort of) years later, and chasing Itachi to see how he is doing.

The night Shisui dies, Itachi awakens the Mangekyou Sharingan. Along with it, he develops a strange ability that allows him to see Shisui, to be with Shisui, but in other contexts… in other lives. As pressure from every corner of Itachi's life begins to close in on him, he finds himself questioning his decisions, loyalty, and what the point of his life truly is if his soulmate is no longer in it. But even in the darkness looming over him, maybe there's hope; a way to find Shisui—and happiness—again.
“Itachi,” Shisui gives him a sad smile, the kind of smile that unnerves Itachi to the bone. “Things aren’t going well in this life, but I will find you again. I’ve seen it. We have more chances, I promise.”
