Just when he felt like he had all he ever wanted, Sanji is suddenly stuck with a glaring reminder of his past, feeling responsible for a brother he rightfully shouldn't give a shit about, and having to confront and communicate his emotions instead of buying them at a crossroads with a stake through their heart. He copes about as well as one might imagine.
