Summary
During an attack at school, Midoriya gets hit by a quirk that leaves him infatuated with the first person he sees, which just so happens to be Todoroki Shouto. Due to an unfortunate side effect of the quirk, Midoriya breaks out into a horrible fever whenever they’re separated for too long, so Todoroki accepts the responsibility of caring for him until the quirk wears off.
Problem is, Todoroki sort of, kind of, maybe likes Midoriya as more than just a friend, which makes the whole situation just a little awkward for him. -

If there’s one skill that Midoriya Izuku prides himself in, it’s being observant.
It makes hero work come naturally to him, since he is constantly on the lookout for things that seem tense or unsafe or even just… strange. Years of bullying along with a sprinkling of a few near-death experiences have honed his instincts, and he’s grown to rely on them when the situation calls for it.
That being said, right now, the situation really doesn’t require sharpened observation skills.
Because, right now, just about anyone could see that there was something off about Todoroki.

Izuku thought that Todoroki wouldn’t be much different during sex. He didn’t cry when Izuku complimented him, he didn’t cry when Izuku said “I love you,” and he didn’t cry when he came- from the overwhelming sensation, or the overwhelming emotions.
Izuku did. Izuku cried at just about anything, so it was no surprise that he cried when Todoroki asked him out, he cried during their first sixteen kisses, and even he cried whenever he fingered himself to completion in the dead of night, thinking about Todoroki’s fingers- or more- being inside of him instead of his own.
He figured Todoroki would be quiet, and relatively unemotional during the act, and leave the crying and desperation to Izuku. He was wrong.

Or: Shouto didn’t get the big deal about sex, until he finally did it with Midoriya.

All Might paled considerably, his gaunt face taking on a look of concern. “The shovel talk? Todoroki, kid, you’re not… You’re not asking for young Midoriya’s hand in marriage, are you?”
“No.”
All Might smiled in relief, opening his mouth to tell Shouto that he was far too young to consider such things, but Shouto beat him to it. “I’m asking your permission to date him.”
All Might spat up blood.
Or, Todoroki seeks Midoriya's dads' permission to date him.
