Maybe he never would. Maybe he would never get the opportunity to explain the way his thoughts had twisted from fear, then rage, then hatred, then radical acceptance, then to some sort of twisted reliance and companionship. How he began to yearn for his touch; in his mind, it was either nothing, or Ralph.
The reliance led to familiarity, and the familiarity led to some twisted sense of love. After all, there had to be something Johnny felt other than hatred if he didn’t want to go insane.
Or, the contents of the "...I'm Gonna Make It Count" ending in which Ralph dies.

Summary
He would defile the very name of his god if it meant he could focus on Johnny, now and forever. To see only Johnny, the beasts of this world be damned, was all he desired.
But he would not get what he desired. Zimforth– Zimforth, and his own consciousness, guaranteed this. His desires were twisted; cruel, corrupted thoughts filtered through his mind day after day, wants that someone as monstrous as him did not deserve to see fulfilled. But the work must be completed, despite everything.
