“Ryuji, is everything fine in there?” he hears a muffled voice.
Obviously, he is not fine. Through the mirror, he can see a slight sheen on his skin, his pale cracking lips, and his disheveled hair. But instead, he urges his best voice to tell Akira that he is fine. That everything is perfect.
Because he needs everything to stay exactly where they are.

or: Ryuji is back on track. Akira moves back to Tokyo. Everything is perfect and back to the way things are supposed to be, until Ryuji realizes that he may be in love with his best friend.

“Are you getting sick? Do you need to stay back tonight? Or maybe we can reschedule…”
Knowing that his face will soon turn redder than a tomato, he places his hands on his cheeks in a desperate attempt to cover it up.
“I told ya, ‘Kira, I’m fine! You worry more than Makoto sometimes,” he says with a half-hearted laugh, hoping that the joke will distract him. “Besides, we can’t reschedule. We were on a roll!”
“Alright, alright,” Akira raises his hands in surrender. “But drink your hot chocolate before it gets cold.”
He takes a sip, and then another, and then another. He flashes the barista a bright grin. “Dude I swear, you probably put some sort of magic in your hot chocolate.”
Akira chuckles and grabs a fresh pot. “Yup, there’s the Ryuji I know. Seconds?”
“Yes, please.”

The words kept ringing in Ryuji's head even after Akira let go. He felt his cheeks warm up, his heart racing as he watched Akira head home from the turnstiles of the train station.
He snapped back to reality as Akira took one last look from the escalator, beaming. Before he could react, he was gone.
or: Akira confesses to Ryuji, and Ryuji runs. Because that's always been what he does best.

Summary
Akira stares down at Ryuji’s unusually still body, his only solace resting in the fact that he can feel him breathing against his side, even if his breaths are short and unusually shallow. He can vaguely hear Futaba and Yusuke on the phone in the distance, as well as the other Thieves’ worried conversations, but Akira can only find himself repeating one thing in his mind.
At least he’s still alive.
OR
Ryuji obtains severe injuries after the explosion in Shido's palace and loses his eyesight.

Summary
“No way… it can’t be.” Akira heard Ann’s faint whimper beside him echo as if it was from another dimension, some other universe where, somehow, Ryuji had fallen, where Ryuji couldn’t smile anymore. “Come on, what the hell. Ryuji… RYUJI!”
It was a bonfire, and his best friend’s twisted silhouette was burned into his eyelids. Akira didn’t realize what he was doing until he was under the water and ice rushed up his arms.
