Summary
The Shadow Proclamation summons the Doctor to correct a grave mistake his previous incarnation made. By not answering Jack's desperate call for help, the Doctor left Jack to an action that has set in motion a paradox that is destroying the Universe. Unless the Doctor can repair his error, everything will collapse into nothingness.

The Doctor and Amy have to get involved in Torchwood's "Children of Earth" arc, reshaping events to save the timeline. Apparently, as horrible as it is, the 456 *HAVE* to make off with a chunk of Earth's children, otherwise humanity never turns their sights to leaving Earth, and humanity dies out. (And that means Jack Harkness is never born on an Earth colony a few millennia down the line, and there's then no one then to stop the 456 in the first place, thus, paradox.) In order to fix the timeline, though, the Doctor has to stop all of his old friends from interfering, first and foremost Jack, so he makes sure a lot of people get captured by the government until it's too late to do much of anything but last-minute scrambling to save *some*. Also, Jack's having a rough time with some temporal bleed-through, "dreams" about the moment of Ianto's death, and in a blind panic sort of accidentally WAY overdoes a desperate Kiss-of-Life and winds up making a totally fine Ianto immortal.
Honestly a lot goes on, and there are a LOT of familiar faces all interacting together in new but awesome ways. Nice and really interesting "fix-it" for CoE. "Happy" ending. (For all of *our* characters, at least.)

"Children of Earth" Fix-It. The Doctor finds the human race dying out, goes to find out what happened to the timeline and arrives post-CoE to see humanity entering another Dark Ages. He asks Martha what happened and she tells him about the 456 and about what happened with Jack--he stopped them (only temporarily, the Doctor knows) at a terrible cost, the Hub was blown up, and his lover died as well. A lover, she tells him, Jack really, *really* loved, and his death broke him. Realizing Jack's lover was near the Hub explosion and therefore got covered in Rift energy, the Doctor goes back to pull Ianto out of the timeline hours before his death, telling him he needs to go to 1965 and stop Jack's exchange with the 456 from happening, because the Rift energy will allow Ianto to change the timeline and avoid a paradox. (Meaning he has to go alone.)
AMAZING exchange between Ianto and past!Jack, as Ianto manages to convince him he's telling the truth and what Jack needs to change. (Incredibly heartbreakingly romantic and sweet how quick their love for each other becomes apparent and Jack accepts/feels it.) Then we see the rewritten/fixed timeline's Jack and Ianto, and both get messages through time (Jack) or the Rift (Ianto) where they try to ensure both the human race's safety, and that they will meet and be together again. Another surprise visit from a very pleased Doctor. Very happy ending.

This story's resolution is beyond satisfying and RoMaNTic aS ShIT. (Ugh, their reunion... It was SO. WELL. EARNED!!!)
Ianto winds up alive but a year in the past before he died, and he has to try to mitigate any potential timeline-altering with his presence, but also kind of needs help figuring out how to "get back to his own time" as the rules of time-paradoxes require--but then what does that even mean for him, since he was dead and outside of the physical world and time? What will happen to him (now alive) if he's sent back? Tosh and Jack have to be made aware of his situation (with a lot withheld), but things are still kind of sad, scary, nerve-racking, and bittersweet while he's there. Things get more complicated when past!Ianto realizes what's going on, and also when future/present!Ianto realizes he can't quite go back with the Rift the way he's supposed to, even with some ripples. But he thinks of a new way--he just has to pray and have faith that his own Jack will figure out what he needs to do in the future. SUCH A FREAKING ROMANTIC AND TEAR-JERKINGLY SWEET REUNION. Happy ending.

(All Martha's POV)
Before the dust has even settled after the fall of Torchwood One, Ianto finds himself signed up with UNIT. He and Martha meet early on (he's her first patient in the field), and then meet several more times, and become really fast friends/family. Martha gains responsibilities for missions after a while due to her experience (with the Doctor) and capabilities, and establishes early on that Ianto will always be her partner or on her team. Thus Ianto is with Martha when they meet Torchwood Three (quite the impression with Jack, he LOVES the UNIT uniforms and red berets, he stood no chance, plus Ianto saves Owen from being killed), so Jack/Ianto still happens quietly in the background. Mostly follows Martha's canon(ish) timeline, with Tom, Torchwood, the Sontarans, the Doctor popping up, and Mickey, just with Ianto always there closely in her life. *Wonderful* chaotic but loving friendship between the two.

Wherever, whenever he is, Jack is still drowning in his grief and misery over what happened with Children of Earth and the loss of the man he'd come to love more than anyone before (and yet wasted every opportunity with and is now left with so many regrets). Luckily for Jack, Twelve--in his final-final moments--decides to pull a Ten and spend his last minutes "righting a wrong" that one of his loved ones has suffered, and goes to Jack. He takes him to the Chamber of the Dead, so that he can talk to the recorded consciousness of Ianto (his memories taken, as with all others, in the moment before his death). They have an emotional (Jack) yet comforting (Ianto) goodbye, and Jack gets to say what he didn't get to before. Twelve pulls him away too soon, but has to make one last stop while he can, and sends Jack out with a device, and incomplete instructions, and Jack realizes he's in a funeral home. Turns out the Doctor recalibrated another Mire healing device, with the one flaw that it renders the patient immortal (like Lady Me).
Happy ending. Heartfelt and sweet.

Ianto's a young timelord, doesn't realize it until TYTNW when time rewinds. Has a BAMF friendship (mostly platonic, just fun) with River, who visits him/travels with him a lot during his time with Torchwood. Jack knows Ianto isn't human, feels some mild angst over that (concerned for Ianto mostly), doesn't learn the truth until after House of the Dead when Eleven shows up demanding to see the other timelord. They get him back from the Rift/Vortex, and he and Jack have to navigate this new element in their relationship (do they still fit?) with Ianto "different" now. Ultimately sweet and happy with a bit of angst thrown in, but strong relationships.
Fic format of 30 linearly-related chapters inspired by different prompts, some cracky, angsty, or fluffy. Ties together fairly well. Fun story and character dynamics with Jack and Ianto mostly staying with Eleven and meeting up with River a lot.

Ongoing snippets of visits between Ianto and Tarot Girl through the events of series 1-3. Ianto and Tarot Girl are the same species--older than they look, and they all have a purpose they can't die before fulfilling, whatever it may be (it's a mystery to them until it's made clear). She's just as enigmatic with Ianto as anyone, but he's more used to it. Children of Earth fix-it as he hadn't fulfilled his destiny yet at Thames House. Jack thinks he's dead and goes to Tarot Girl in desperation for a way to at least see Ianto one more time, but lo and behold Ianto's already there. Happy reunion, conversation, and happy/hopeful ending. Ianto had been told before he was made "to love and to be loved", and later that Jack played a part, maybe. Now Ianto's pretty sure his purpose is to do so with Jack--whiiich, given Jack's own long lifespan, likely renders Ianto essentially immortal as well. (He certainly can't die on Jack early in his timeline and leave heartbreak instead of love, now can he?)

Daemon!AU mostly following canon events, but yet SO MUCH is still changed as a result of this universe. This Ianto knew Lisa was gone when he found her since her daemon was gone, and really just accidentally has all those run-ins with Jack that ends up with his hiring. They get together (in whatever aspect they start as) right away. And their daemons gets along so well along with their own dynamics fitting, that Jack is a lot quicker to commit happily to the idea of just being with Ianto. (Ianto's literally the only person, including past loves, that Jack's daemon allows physical contact, and Jack *recognizes* that as being incredibly significant.) They actually date, there's no drama with Gwen in that regard, Owen and Tosh date (before they die), and while CoE still happens and Ianto dies, the events of Miracle Day are altered to revolve around daemons and Dust, and through various means (including Bad Wolf), Jack gets Ianto (and Ianto's daemon) back eternally.
Happy ending, REALLY cool adaptation of the daemon-trope and created world, and much happier and more satisfying character relationships and dynamics. Great read.

After Jack's finished with his reunion with the Thirteenth Doctor and the whole Dalek problem, Gwen's son sends a message telling him he NEEDS to get to them, as soon as possible, but won't specify. When he gets to Gwen (who despite everyone's best efforts over the last few years wasn't able to get ahold of Jack before), she tells him Ianto's back, and is on his way to meet them after leaving Martha and Mickey in London. Ianto came back, and seems to be the same kind of immortal that Jack is (left unexplained, but not the point). They get to have their reunion, and it's all very sweet as Jack clings and cries, and tells Ianto he loves him. Unclear where they'll go, but it IS clear that whatever they do, Jack's making sure they do it together.
Very lovely--both the reunion and seeing the large family/network Ianto's created for himself on Earth upon his return.

PART 1:
Post-Canary-Wharf, Ianto's about to commit suicide when the Doctor shows up, led by a message for help on his psychic paper (later revealed to be from a dying Face of Boe). The Doctor saves Ianto and takes him with him, as a companion. Grief is a profound theme, and a constant battle even on Ianto's good days. He does amazing things, then leaves and joins Torchwood Three, falls in love with Jack mostly as canon goes. He's insecure and, while amazing, always thinks he means nothing, to and *next to* the two most incredible people in his life (an immortal and a Time Lord). The Doctor's there at Thames House, but trapped in his TARDIS and forced to watch the feed of Ianto's death. Thus it ends with Jack's and the Doctor's profound grief, worried Ianto didn't know how important he truly was to them. (BEAUTIFUL STORY, but you *WILL* cry your eyes out!!!)PART 2:
A fix-it for part 1. Danny uses his one free life to send Ianto back instead of himself (or the boy in canon), asking Clara to look after him. His memories are confused at first and things are a bit awkward, but he and Clara become great friends. Then he catches her with the Doctor one day and they have a (not super great) reunion, and he demands to be taken to Jack. They reunite, go back to live a life together on Earth, join UNIT under Kate, Ianto dies AGAIN, and Jack waits for the Doctor to come, him and Clara demanding he FIX IT. Luckily the Doctor was going to give them a Mire repair kit as a wedding gift, now it's just given early. -->Immortal!Ianto.

Ianto disappears after Jack goes too far in resurrecting Owen, with Ianto scared of what else Jack will do when the people he cares about die--when *Ianto* dies... He lets them believe he Retconned himself, and they can't find him, despite desperately looking (Jack and Tosh don't give up... they just need to know he's okay). Ianto faked it and actually joined UNIT as an undercover agent with a handler, and he's totally BAMF. He reveals himself after learning of the assassination orders and calls Jack to warn of the bomb, which still blows up, but Gwen, Mickey, and Martha (who joined up to help after T3's staff losses) escape. Ianto and them are on the run, he rescues Jack and things proceed differently than canon, with Ianto's contacts opening new doors for means of help and options available. Ianto goes to the Black Archive and meets the Eleventh Doctor (who helps him just a *tiny* bit in escaping) when he goes for a piece of cybertech that will allow Jack to stop the 456 rather than use a child. SO MUCH ANGST and DRAMA for everyone, and people's feelings are rough, but it's so clear just how much Ianto and Jack still love each other. Jack wants to be with Ianto, with all the clarity in the world now, but Ianto's insecure and his reasons for staying apart don't change.
Ends with them reuniting and starting a "happy ending" for them after Jack closes the Rift and becomes mortal--which gets rid of Ianto's primary reasons for breaking up. The Doctor pays Ianto a visit, and talking to Jack clears up the last of his insecurities. Time and space is open to the two of them to start a new life, together.

A snippet of a CoE Fix-It. Emotional and all through Jack's perspective as he wakes from his death and realizes that Ianto's gone, and then goes through his initial, unbearable grief as he laments about just how MUCH Ianto actually meant to him, despite him fooling himself with a charade of casualness or distance to protect his heart--he fell in love with Ianto anyway (he *hurts*, anyway), and it's all the worse for him having missed his chance to even *tell* Ianto that. But then, as he lightly kisses Ianto a final goodbye, he feels a sudden breath on his lips...
Sad and angsty, but the cathartic, satisfying kind, and has a happy ending.

Fix-It fic, everybody lives. Snippets in time from Post-Cyberwoman to CoE, as Jack reflects on how easily he falls in love with Ianto, and the fear, uncertainty, and profoundness that comes with it. He wasn't prepared for just how much he would love him.Little bit of angst, temporary character death in CoE, but happy ending.
