Mature rating is for Mature subject matters, almost no sex. This not sexually oriented, though there is a brief moment interrupted.
Kathryn Janeway has now been back in the Alpha Quadrant for over eight years now. She suppressed so much while in the Delta Quadrant, despite allowing Chakotay in for brief looks into what went through her mind, but she never actually dealt with any of her feelings and self-talk. Still blaming herself, degrading herself, and more. Despite going through postpartum counseling, she still never dealt with the Delta Quadrant. Things got worse, finally abandoning her children with Rhianna and Chakotay, she returned a year later and accepted Fleet Admiral at the Betazed Defense Force, which was seen in "If You Love Someone..." https://archiveofourown.info/works/64520572 . She also did some weird things in another story too. Things take a turn for the worse a few months after she finally retuned to her family with Deanna ordering her to a psych eval.
