Summary
Garak continues the process of recovery from the consequences of the failure of his neural implant - and of dependence upon the narcotic drug triptacederine - with the help of Doctor Bashir, Counselor Telnorri, and Nurses Heyns, Patel, and Jabara - and with the support of Keiko O'Brien. He is trying to regain his independence - though the idea is both important and frightening. And, he is attempting to return to his profession on a full-time basis - while he and Professor O'Brien continue their work on the station's new Arboretum.
Meanwhile, Dr. Bashir continues to try to be a good friend to Garak, not merely a doctor; Keiko tries to find a balance between encouraging Garak to occupy himself with his hobbies while clarifying that she doesn't expect him to accomplish everything all at once; and Counselor Telnorri strives to adapt to the new role of counselor-to-someone-who-doesn't-truly-want-counseling – and who is no longer required to participate in mandatory counseling sessions.

Quark's Bar was no less crowded or noisy than Garak had expected it to be. The lights were no less bright, the inescapable onslaught of conflicting scents no less malodorous, the temperature no warmer.
Stepping inside would be unpleasant. Remaining there for sufficient time to place their orders, obtain their food, and finish their meal was certain to be nearly intolerable.Yet Julian would know the reason, if he were to make an excuse - and besides, Julian was there. Surely he would offer a delightful distraction in the form of conversation - and perhaps even argument.

Julian helps the changeling impersonating him.
It wasn’t like the other time, on the Founders’ planet. The connection was much more direct, and there was no point in hiding what was happening. He didn’t have exact control over his- its body. He couldn’t have controlled it anyway; the signal was too laggy, throttled down to a trickle so it could slip through the wormhole disguised as background noise.
Still, it let him leave his quarters and play darts with Miles.
He considered spelling out some sort of message. Nothing overt, nothing that would be obvious in one meeting. Perhaps clustering the darts to one side, or in some other odd-but-not-impossible pattern. The clacking of the dabo wheels stuttered across the connection. Miles lurched from one position to the next, not quite as smooth as he would be in person. He didn’t even know if this was happening in realtime, or if it was some composite scenario the changeling was researching for later. But he could see Miles, and Leeta, and Morn, and even Quark, and they were alive, and the station was peaceful and he was home.

Once upon a time, Julian imagined he might do better on a planet other than Earth, that he was a sort of modern-day changeling child, abandoned on a planet he wasn't fit for.
It cut at the time that it wasn't true - it cuts all the deeper when, some years down the line, Earth sees fit to abandon him to someone else's planet after all.

As he had explained to Garak when he handed over the invite a few lunches earlier, Julian had for some years done little to observe the occasion at all, following on from an unpleasant birthday experience in his formative years. Now, however, he had chosen to seize the opportunity to celebrate on his own terms. An admirable sentiment, no doubt, but it did place Garak in the unenviable position of having to hobnob with most of the station’s senior staff, and he was very confident that most of them did not like him nearly as much as Julian did.

These damn Cardies, Miles had said. Put me through all those medical procedures when they’re planning to execute me anyway. Didn’t have the decency to just kill me.
Something in that conversation had stuck with Julian, and was stubbornly refusing to get unstuck. It was only that… well. That part of Miles’ report, with the chair and the samples and the holding his head still, had put him in mind of some difficult patients he’d had in the past. One thought led to another, and he’d started picturing a very particular difficult patient under those too-bright lights. Not in the chair, exactly, or maybe, if he could be wrestled into it?
None of this was anywhere near appropriate to voice aloud, which is why he was halfway through the question before he realised he’d opened his mouth.
