Story is told from POV of the bottom. He is a trans man but there is not really any indication of that. My feeling is that this plot and the two people involved only makes sense as two men. But, due to the lack of sexual anatomy mentioned (for both) and the lack of gendered language (re: bottom), it could be read with a different combination in mind.
Story is focused mostly on the bottom's asshole and what is beyond, with occasional appearance of facehole. Even though it's basically a rather long enema story, there's no poop.
