“Oh, Sanji,” She turned on her heel with a smile plastered on her face, while Sanji felt himself only becoming more confused. She started slowly walking back around the countertop, turning her head to face the cook once more. “I didn’t do anything. I haven’t changed anything in here, in fact. You have, though.”
He stared at her incredulously as she paused. Sanji could’ve sworn her eyes looked him up and down, and he didn’t want to think about what that meant.
“What do you mean I have? What have I changed?” His words came out in a pleading tone towards his new acquaintance.
“You’re the director of this whole thing.” She gestured around her, her face looking like she’d just told some sort of silly joke that Sanji wasn’t in on. “This is your dream, after all.”

Sanji's dreams have been infiltrated by a very cocky, very pretty, and kind of annoying figment of his imagination.
When he sees her again - this time in front of his very real, very awake self, he can't help but wonder if she was ever a figment at all.

Impatiently, Zoro tugged at Sanji’s shirt and Sanji let Zoro take them off. His rough fingers reached down to the belt once again. As Sanji listened to the sound of the metal buckle and the zip of his pants being undone, it took everything in him to not push Zoro away and bolt. Instead, he watched his trousers being tossed aside and dragged his eyes down to Zoro, who was watching him like a hawk.
“You look as if you’re walking up to your gallows,” Zoro noted.
“An apt description,” Sanji croaked.
Zoro frowned in curiosity. He turned his eyes back down to the blue-striped boxer before reaching for the hem and slowly peeling it down. Judging by Sanji’s utter apprehension, Zoro was expecting something hideous, like a scar or a deformity, but he didn’t expect this.

Summary
During the Straw Hats' last night in Wano, Sanji spends his night drinking and overthinking how to tell his crew about his relationship with Zoro when Kid happens upon him. After accepting a drink from Kid but rebuffing his advances, Sanji starts to feel unwell. Not wanting to worry his crewmates, he accepts Kid's offer to help him back to the Sunny, a mistake that will devastate Sanji and leave the Straw Hats reeling. As Sanji spirals deeper into depression, Zoro struggles to make sense of what the hell happened, and how he can keep Sanji-- and his world-- from falling apart.

The man's smile grew to show his teeth, "Then you work this alley? I've heard good things."Or: The one where Zoro gets propositioned for money (that he desperately needs), so he agrees to something (that he doesn't understand) ... And Sanji sees it all.

Summary
Exhaustion was beginning to become a constant companion to the poor cook of the Straw Hat Pirates. Sanji hadn't slept in weeks. Even after Wano, the memories of his childhood as the youngest child of the Vinsmokes has begun to haunt him through every night. Constantly woken by the visions and sounds of his family trying to destroy his nakama with beatings and hollow words, there was only so much his broken mind could take.
Unfortunately, Sanji stumbled on one solution that helps. But, no matter what, he's not willing to put himself through it. Cuddling with a crewmate was one thing, being held by Zoro of all people was another. So, deciding that he'd take some time on their next island to sort his head out and find some rest, Sanji was not ready for what was in store for their crew.
A new island only meant trouble and life altering decisions that could mean the end of Sanji's lifelong dream, or the beginning of a brand new adventure.
