Excluding two short stops at rest areas—quick and discreet, much more so than in the days when they were hunted by the FBI—they had been on the road for eleven hours straight.
Eleven hours with the sole objective of hiding away in this lost corner of Alabama.
No, it wasn't a planned destination from the start—but to Dean, it felt far enough from the Northern Indiana General Hospital, so it was more than fine. -

He banked everything on his puppy eyes. Or rather, he tried to mimic Sam’s. His brother could always get the impossible from the unlikeliest of people with those, so why shouldn’t they work now?
He mentally celebrated when she, after scrutinizing him a bit longer, turned away, exasperated, mumbling as she disappeared among the shelves in search of something. She returned with an irritated look on her face and what would determine Sam’s fate in her hands: three boxes.

"I don't feel so good—" Dean had figured that out on his own, but hearing it from Sam hits differently. It's the sound of defeat, the most complete collapse.
Sam can't 'not feel good.' He's not made to 'not feel good.' Not in front of Dean's eyes. Dean stretches his lips into a crooked line, takes a deep breath through both nostrils.
The strange mechanism that makes him take responsibility for all of Sam's 'not feeling good' has been active for a while (and this time, he's right).
He silences it as best he can, cursing it and cursing himself. A big, messy, endless internal turmoil, the only visible echo of which to Sam is the slight tremble of the hand with which Dean wipes his forehead.
He puts away the bandana when Sam's back seems to have settled too comfortably on that dirt road. Dean stands up, sniffs. -
