The multiversal spell meant to remove Otto from existence doesn't, leaving the reformed villain alone in limbo, and with many questions, beginning with Jameson's newest broadcast about the unmasked webslinger.
Last time Otto checked, Peter was anything but unmasked.

A smile curls his lips, the kind of smile that’s gotten him into trouble more than once. It’s sharp, knowing, and just shy of wicked. “Doctor Octavius,” he says smoothly. “A pleasure.”
Octavius does not smile back.
“You’re late.”
“By thirteen minutes,” Stephen points out with an arched brow. He taps the face of his own watch, purely for effect and, perhaps, to show it off a little. “I’d call that fashionable.”
Octavius doesn’t even blink. “This project is operating under precise timelines. If you can’t respect that, you can leave now.”
A chuckle is Stephen’s initial answer, low and easy, as if Octavius has just complimented him. He takes a languid step forward, as though daring the man to make good on the threat. “Feisty, aren’t we?”
