A frog jumps out of boiling water, but heat it slowly? The frog won't try to escape until it's too late.
Lord Cazador Szarr saved him the night of the Gur attack. He took him in as a ward and offered shelter and protection. By the time Astarion realised what life as a vampire's spawn really meant it was too late to run.

What happens if they lose to Cazador?
...
When he had finished, he sat on the edge of his bed and waited. Silent and motionless.
He waited, not daring to do anything else.
The sun rose and set again.
It repeated itself. And even after the fourth repetition, he still sat there motionless.
Astarion thought about the others. Without water, they would slowly die of thirst. And hunger would gnaw at them too.At the fifth sunset, Astarion's door opened. He remained seated and waited for instructions. The vampire didn't look up either. He knew which of his siblings was standing there. Cazador always instructed Petras to fetch him.

Summary
“I am sorry, Master,” Astarion said finally, then was silent. He would not betray the name of his beloved by uttering it here. He would not give Cazador the satisfaction of spitting it back at him, tainted.
Cazador waited several long minutes, perhaps wondering whether Astarion would break under the pressure of his silence. For once, Astarion was tired enough to outlast his own fear, and still did not speak. Cazador owned him, body and soul, but unless he compelled him now, he would say nothing more. He wondered, absently, why Cazador didn’t compel him to grovel, to reveal all; but instead he just stared at him, a dour expression of disappointment on his face.
“Not as sorry as you shall be,” Cazador said at last, schooling his features as he slowly turned to greet the first guests arriving for the nighttime garden party, “if you cannot bring every guest here to the peak of pleasure before sunrise. I think even your wits are not too dull to understand the consequences of failure in this.”
